Job Description

Position:  Pharmacy Transition Specialist

Location:  Imperial Center / Durham

Hours:  Full Time, 40 hrs/wk; Days


This position may involve support of various hospitals and health care systems within the UNC Health Care System, but will be employed by Rex Hospital, Inc. (this includes, but is not limited to, for purposes of payroll, health benefits, retirement options, and applicable policies).


Key Responsibilities:

  • Complete pre-certification for all patients referred for medical or pharmacy benefits review: include
  • Investigate options for supplying or replacing medications received by self-pay patient

  • Maintain current knowledge of, requirements for and ability to enroll patients in: manufacturer, state and local medication assistance programs; disease based assistance from non-profit organizations ; Medicare prescription drug benefits; Social Security Low Income Subsidy; state and federal medication subsidy programs

  • Work collaboratively with other health care professionals to obtain clinical and financial information needed for program enrollment

  • Utilize internally developed systems, the electronic health record and commercial vendor software to track patient eligibility, prescriptions, patient communications and program enrollment

  • Document and communicate findings to the patient, provider and pharmacies per internal operational standards.

  • Evaluate all self-pay patients for PAP eligibility and enroll patients as possible

  • Educate staff, prescribers, patients and their families regarding program requirements and payer restrictions

  • Understand and interpret insurance terms to appropriately triage referrals

  • Communicate regularly with physicians, pharmacists, nurses, and other staff to ensure customer and department staff needs are met

  • Interact with physicians, nurses, and other health care professionals in a courteous and professional manner

  • Work cooperatively and collaboratively with scheduling, financials counselors, pre-arrival, pharmacy, physicians, nurses and other staff and management to provide a professional and cohesive work environment

  • Proactively seek solutions to customer problems encountered both inside and outside of the department

  • Screen for and/or identify barriers (literacy, language, compliance, etc.) to medication management/access and refer patients to appropriate nursing/pharmacy staff member

Qualifications

Completion of a nine-month Pharmacy Technician Program (or equivalent combination of education, training and experience). All new hires not currently enrolled in a pharmacy school are required to have their certification as a pharmacy technician. 

 

If completion of a Pharmacy Technician Program: No prior experience required.

 

If a High School diploma or GED: Two (2) years of experience as a Pharmacy Technician.
