We are hiring for a Supervisor of Population Health. The Supervisor is responsible for supervision and delivery of care management services in a variety of settings. Develops collaborative relationships with clinicians and staff to facilitate improved patient outcomes. Identifies and addresses barriers to care coordination and efficient workflows. Accountable for achieving productivity and quality targets. This position formally supervises employees. Other duties may be assigned.
BSN, MSW, or related health care degree required. RN, Fully Licensed LCSW, or appropriate licensure for field of study in NC required. Three years of clinical experience in an acute care or ambulatory care setting caring for patients with complex medical conditions required. Experience leading or participating on a multidisciplinary team required. Master's degree preferred.
Excellent interpersonal skills and communication capabilities. Knowledge of care management and quality improvement principles. Possess appropriate analytical skills to independently problem solve and make decisions. Possess computer skills at a level sufficient to effectively and efficiently support patient care.
