Job Description

Maintains an environment of safety for patients, self and others.

Evaluates patients and their environment to include: medical history, strength, motor development, mental status, sensory perception, mobility, functional capacity, respiratory and circulatory efficiency, safety issues and DME recommendations. Records findings to develop or revise treatment programs.  Develops care plans based on past medical history, evaluation of patient data, patient/caregiver goals and physician orders.

Acts as case manager to coordinate delivery of home care services. Develops and revises the home health aide's Plan of Care to meet the individual needs and goals of the patient.

Provides therapeutic exercises, administers physical agents, recommends home adaptations and equipment, instructs patient/ caregiver in functional mobility and home programs.

Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit. Documents evaluations, treatments, patient response and progress in patient's chart.

Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate physical therapy treatment with other aspects of patient's health care.

Communicates effectively with patients, families and other staff members. Notifies patients and schedulers and/or team leader's of schedule changes. Orients and instructs activities of aides in relation to patient care plan. Participates in performance improvement activities and committees as assigned.


One to two years previous related experience desired. Bachelor's degree (B. S.) from four-year college or university preferred.


Licensure *Must have NC PT Licensure.
