Overview:
The Department of Pharmacy is seeking a highly motivated, energetic, and dynamic individual to serve patients across the transitions of care continuum. The Pharmacy Transition Specialist working with the Carolina Care at Home Meds-to-Beds team will be responsible for logistically organizing, collaborating and/or delivering required discharge medications to patients and/or families or caregivers. The Specialist will ensure the successful hand-off to the appropriate medications to enable the improvement of the patient’s health management and experience.

The Specialist will interact primarily with physicians, pharmacists, other members of the patient care team, as well, as work closely with members of the outpatient pharmacy team. 

Responsibilities:

  • Interact with patients to obtain permission to fill discharge medications, ensure patients are eligible for services, have method of payment, and are properly educated 
  • Deliver discharge medications to patient’s bedside in a timely manner 
  • Responsible for the procurement of all patient’s information and prescriptions prior to discharge 
  • Coordinate with clinical care team, case management, and other staff to ensure medications are delivered without delaying discharge 
  • Demonstrates willingness to be a team player through mutual respect and cooperation with others 
  • Flexibility in work assignments and adaptability as patient and hospital discharge needs change 
  • Assist team members with medication deliveries and benefits investigations, as needed 
  • Maintains a professional image and conducts behavior appropriate to the situation 
  • Maintains a neat personal appearance 
  • Understands nuances of pharmacy insurance claims, medications associated with different disease states, and pathways for patient or pharmacy assistance 
  • Has basic understanding of a hospital health care system and its component parts, including sites of care, delivery models, and the roles of various providers and health care professionals 
  • Comfortable working autonomously 
  • Ability to think broadly and longitudinally, constantly assessing and anticipating the needs of the patient and his/her environment 
  • Effective communication skills 
  • Participates in a culture that embraces creativity, productivity, communication and quality improvement 
  • Ensures the Carolina Care experience is consistently delivered to every patient 
  • Ensures all operating policies and procedures are followed at the highest level 

Preferred Skills and Attributes:

  • Experience as a pharmacy technician in an ambulatory, outpatient, or retail setting 
  • Customer service experience 
  • Written and verbal communication skills 
  • Ability to both lead and collaborate as part of on-going team participation 
  • Ability to prioritize and self-manage workload 
  • Ability to analyze information and identify inconsistencies 
  • Continuous problem solving and quality improvement

Education Requirements:
● Completion of a nine-month Pharmacy Technician Program (or equivalent combination of education, training and experience).
Licensure/Certification Requirements:
● Registration with the NC Board of Pharmacy unless enrolled in a PharmD program.·CPhT from PTCB or ExCPT from NHA unless enrolled in a PharmD program.
Professional Experience Requirements:
● If completion of a Pharmacy Technician Program: No prior experience required.
● If a High School diploma or GED: Two (2) years of experience as a Pharmacy Technician.
