This opportunity is for a part time Care Manager who will be part of an integrated behavioral healthcare team. The Care Manager will function as a core member of a Collaborative Care team that involves the patient, the Primary Care Provider (PCP), Psychiatrist, and other specialty providers in the clinic. The Care Manager is responsible for supporting a coordinated approach to care for patients with behavioral health conditions.

Summary:
Completes clinical assessments of bio-psychosocial needs, provides information and short-term therapeutic counseling to patients and their families in relation to social, psychological, financial, and family situations and prepares an intervention plan. Obtains, interprets, and communicates to staff and physicians necessary information regarding patients and families, including bio-psychosocial history information. Identifies possible or actual patient/family crises, providing crisis intervention and emotional support, and creating and facilitating effective plans for resolution. Develops and maintains good working relationships with outside community health and social agencies. Reaches out to the community to learn of and utilize new resources for meeting patient needs. Assists patients and families with obtaining community assistance by referral to proper resources. Documents group therapy and other care activity in patient charts according to department policies and procedures. Adheres to National Association of Social Workers' Code of Ethics.

Principal duties will include:

• Support and closely coordinate mental health care with the patient’s primary care provider (PCP) and, when appropriate, other mental health or specialty providers.

• Screen and assess patients for common mental health and substance abuse disorders.

• Provide patient education about common mental health and substance abuse disorders and available treatment options.

• Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.

• Support psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment.

• Provide brief interventions using evidence-based techniques such as Behavioral Activation (BA), Problem-Solving Treatment (PST), Motivational Interviewing (MI), or other treatments appropriate for primary care medical settings.

• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g, Cognitive Behavioral Therapy (CBT), Interpersonal Therapy (IPT)) as clinically indicated.

• Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s PCPs.

• Track patient follow-up and clinical outcomes using an electronic registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.

• Document patient progress and treatment recommendations in the electronic medical record so they can be easily shared with IBD providers, the psychiatric consultant, and other treating providers.

• Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCPs and the psychiatric consultant. These may include changes in medications or psychosocial treatments or appropriate referrals for additional services.

• Facilitate referrals for clinically indicated services outside the clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

• Complete relapse prevention plans with patients who are in remission.

● Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education.
Licensure/Certification Requirements:
● Licensed as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate (LCSWA) in the state of North Carolina.
Professional Experience Requirements:
● No prior experience required.
