Summary:
The Nursing Care Coordinator within the Pharmacy Department Medication Assistance Program (MAP) will work closely with pharmacy staff, clinic staff, and providers to obtain the appropriate clinical information to meet insurance requirements and manage prescription medication insurance appeals for the MAP Pharmacy Benefit service line. The MAP Nursing Care Coordinator will also be responsible for providing patients and clinicians with accurate and up to date information. This position will report directly to the Clinical Manager over Medication Assistance Program Pharmacy Benefit Services.

Responsibilities:

The primary job responsibility includes managing prescription medication insurance appeals for the MAP Pharmacy Benefit service line and serving as a clinical resource to other MAP specialists during the benefits investigation process and assessment of medication assistance for patients. Other duties include one or more of the following care coordination, data analysis and performance metrics, and education:

1. Assesses patient status of a specialized population to determine and coordinates on-going patient care needs related to medication assistance. Uses expertise and experience in specialty practice to facilitate patient care. Acts as a liaison between the patient, family and health care team to communicate patient care needs related to insurance requirements and medication assistance.

2. Collaborates with the health care team to provide information and resources to facilitate patient care.

3. Collects data related to specialized patient populations. Enters data or coordinates its entry into databases and registries for benchmarking purposes.

Promotes own professional development through attending presentations. Keeps current in practice through conferences, reading current literature and research. Role models professional nursing practice to others through use of professional behaviors.

Education Requirements:
● Graduation from a state-accredited school of professional nursing
Licensure/Certification Requirements:
● Licensed to practice as a Registered Nurse in the state of North Carolina.
Professional Experience Requirements:
● Five (5) years of professional nursing experience.
