Responsibilities:
- Manages human resources for population health sr. leader, social workers/care managers and MedServe Fellows and creates an environment conducive to staff recruitment and retention
- Provides supervision and consultation to the social workers/care managers and MedServe Fellows
- Conducts annual performance reviews of staff
- Provides and facilitates an environment conducive to staff continuing education needs, on-going unit-based educational needs and orientation of administrative and clinical staff members
- Accountable for annual competency evaluation of staff
- Responsible for coordination and evaluation of population health management activities including patient outreach, patient education, behavioral health, quality improvement, and care transitions
- Responsible for the business development and expansion of embedded care management, chronic care management, behavioral health care management, collaborative care management and care transitions
- Participates in quality improvement initiatives to address identified process or standard work issues
- Represents population health programming internally within the Department and maintains Family Medicine representation on Healthcare System workgroups
- Monitors quality metrics related to total cost of care, ED utilization, 30 day readmissions, enrollment in enhanced programs, and preventative screening measures
- Participates in residency teaching, clinic staff development, and student internships
- Engages in research related to population health initiatives for presentation and publication
- Supports FMC operational needs as a member of management team
- Acts as behavioral health crisis support within the Department
- Liaisons between community medical and behavioral health agencies and the Department
- Responds to reporting needs from Department leadership and Health Care workgroups, gathering, analyzing, and interpreting data to inform program development and evaluation
Other Information:
Education Requirements:
- Bachelor’s degree in Business Administration, Health Administration, or related field (equivalent combination of education, training, and experience).
- Master’s degree in Business Administration, Health Administration, Nursing, or Social Work is preferred
Licensure/Certification Requirements:
- LCSW or RN licensure preferred
