Summary:
The Solutions Designer serves as a liaison between clinical and operational stakeholders to plan, develop and deliver Population Health EMR-based and other solutions, in collaboration with ISD teams and other system stakeholders. The Solutions Designer collaborates with stakeholders to understand, clinical and operational needs and translate those needs into EMR-based and other solutions. They leverage knowledge and expertise in EMR population health capabilities, as well as data/operational/clinical considerations and implications to deliver solutions. They are responsible for planning and project management needed to successfully deliver a fully functional solution. This position will focus on clinical and operational solutions related to value-care populations and interoperability with independent clinics within our accountable care organization.
Responsibilities:
Requirements Gathering: Analyze clinical, operational workflow and data requirements to translate into requirements for ISD and other Health Alliance Teams. Leverage knowledge of existing tools and functionality, and develop new capabilities via innovative solutions. Prepare requirements for agile process and update project schedules based outcomes of agile sprints. Work in close collaboration with ISD Teams and other Health Alliance teams to ensure progression of work and to address barriers.
Project Management: Responsible for planning and execution of projects to deliver solutions, in collaboration with clinical/operational stakeholders, ISD team and Health Alliance colleagues. Develops charters, project plans, goals, schedules. Identifies, troubleshoots and communicates project barriers and risks. Manages multiple, multi-disciplinary, cross-departmental projects.
Analysis & Consultation: Provides consultation and support to internal and external stakeholders related to existing solutions and capabilities. Leverages knowledge of clinical and external data integration and interoperability.
Quality/Process Improvement & Outcomes: Utilizes quality/process improvement tools and methodologies to understand business needs, ensure use of tools/solutions is resulting in improved processes, improved patient or provider outcomes, contributing to key strategies for improving value-based care performance (ie improves quality, lower cost, reduce care variation, etc). Takes learnings from projects and collaborations, and applies quality/process improvement tools to continuously improve our ability to manage our work and deliver solutions.
