Summary:
The Social Worker is responsible for performing a wide range of patient care activities for assigned patients. The Social Worker conducts patient assessments, contributes to the patient treatment plan, and the discharge plan; and provides individual, group, and family counseling as indicated. Adheres to National Association of Social Workers’ Code of Ethics.


Responsibilities:
• Completes a biopsychosocial assessment of patients; assists patients to develop a discharge plan; obtains collateral information from family members, community providers and other supports as appropriate; provides crisis intervention and counseling as needed to facilitate a safe plan of care; serves as a patient advocate, representing patient’s interest to hospital team.
• Assists inpatients and families prepare for discharge through coordination of care, referrals to appropriate services, education and consultation regarding resources, and placement as needed. Identifies possible or actual obstacles to safe discharge and provides intervention to resolve these issues. Serves as patient advocate to secure adequate community services support and/or appropriate continuing care. Demonstrates proper performance of skills reflected and validated by the unit specific orientation/skills checklist.
• Develops and maintains good working relationships with outside community providers and social agencies, collaborating as necessary regarding discharge plans. Reaches out to the community to learn of and utilize new resources for meeting patient needs. Participates in community outreach and education efforts as appropriate. Assists patients and families with obtaining community assistance by referral to proper resources.
• Obtains, interprets, and communicates to staff and physicians necessary information regarding patients. Maintains communication with patent’s family members and/or significant relationships as indicated throughout the course of hospitalization.
• Provides regular and consistent participation in assigned hospital committees and duties, representing the profession of social work as well as the PATHS unit. Adheres to policy and procedures and maintains social work licensure. Adheres to the Code of Ethics of the National Association of Social Workers.

Qualifications

Required
• Masters of Social Work degree
• Licensed and certified as a Licensed Clinical Social Worker (LCSW) in the state of North Carolina
• Experience performing individual, group, and family counseling
• Experience performing discharge planning
Preferred
• Two (2) years experience as a clinical social worker in a healthcare setting
