Summary:
The Pulmonary Disease Navigator (PDN) utilizes clinical judgment and expertise to coordinate care management activities for adult pulmonary patients. The PDN strives to ensure that patients and caregivers are clear on when they should be contacting their pulmonologist or primary care provider and why it is so important that they are making those outreaches early and not waiting. 

The primary responsibilities of this position are to perform patient assessment, develop and implement treatment plans, provide education to both patients and their families, and assist in discharge planning prior to discharge. 

  • This individual will work closely with the facility’s interdisciplinary team (providers, case management, nursing) through rounding and through both verbal and written communication and collaboration. 
  • This individual will serve as a liaison between pulmonary medicine and the interdisciplinary team. 
  • This individual facilitates seamless care for adult pulmonary patient (outpatient and/or inpatient) and their families and ensures effective and timely use of resources to address identified needs. 
  • This individual promotes quality care delivery across the continuum of care. 
  • Arranges referrals to pulmonary rehab, pulmonary function testing and sleep diagnostic testing

 Additional duties include, but are not limited to:

  • Admission Assessment for patients with diagnosed or suspected pulmonary involvement. 
  • Nursing Education may include any combination of the following, based on facility- and patient-specific needs, including, but not limited to: 

  1. Pulmonary Disease Education 
  2. Symptom Management 
  3. Condition exacerbation prevention/intervention
  4. Education regarding when to send to ER/consult with physician 
  5. Early identification of signs and symptoms of condition exacerbation 
  6. Breathing Exercises/Respiratory Treatment interventions 
  7. Medication review and administration methods 
  8. BiPAP and/or CPAP Education

  • Patient/Family Member/Caregiver Education regarding pulmonary/respiratory diseases and conditions and pulmonary health and wellness, including, but not limited to: 

  1. Pulmonary Disease Education 
  2. Symptom Management, including education on when to send to ER/consult physician 
  3. Condition exacerbation prevention/intervention 
  4. Early identification of signs and symptoms of condition exacerbation 
  5. Breathing Exercises/Respiratory Treatment interventions 
  6. Medication Management Education 
  7. Smoking Cessation support
  8.  BiPAP and/or CPAP Education 

  • Pulmonology/Primary Care Physician communication and outreach to establish protocols, report patient progress, and establish rapport.

Responsibilities:
1. Implements appropriate pulmonary education plan according to the individualized needs of the patient with full consideration of patient safety needs.
2. Supervises the activity of the clinical staff.
3. Provides objective performance evaluations of the clinical staff.
4. Coordinates clinical staff scheduling.
5. Coordinates and supervises orientation of new respiratory care personnel.
6. Assists in the determination and implementation of cost effective procedures.
7. Recommends new policies and procedures, and revisions of such to the manager.
8. Responsible for the clinical proficiency of the respiratory care staff.
9. Coordinates cardiopulmonary continuing education events for the respiratory care staff and allied health personnel.
10. Participates as a cardiopulmonary resource liaison in hospital related committees, training sessions and other activities.
11. Maintains accurate department records.

Education Requirements:
● Bachelor's Degree required within 3 years of hire. Prefer candidates with Bachelor's upon hire.
Licensure/Certification Requirements:
● RRT with adult and neonatal intensive care experience required. ACLS , BCLS , NRP , PALS , RRT
Professional Experience Requirements:
● Requires three (3) years of previous related experience in the field of respiratory therapy.
Knowledge/Skills/and Abilities Requirements:
● LANGUAGE SKILLS: Ability to read, analyze, and interpret clinical information, professional journals and technical procedures. Ability to document clinical information. Ability to write reports and procedure manuals. Ability to effectively communicate information and respond to questions from patients, physicians, family members and groups of managers. MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry. REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
