Summary:

Under direct supervision of a Registered Pharmacist in the Emergency Department, performs medication reconciliation for patients in the Emergency Department.  Incumbent also notifies pharmacist of any discrepancies between patient problem list, medications from home and medication list in the EMR. 

Responsibilities:

  • Successfully performs medication reconciliations.
  • Identifies patient safety issues within the medication reconciliation process
  • Maintains ED floor stock medications/supplies in ECC Pharmacy.
  • Communicates with team members and other care providers regarding assigned duties.
  • Acquires age-specific knowledge necessary to provide appropriate service and communications.
  • Maintains a good and current working knowledge of the clinical computer system.
  • Participates in maintaining designated equipment and work areas.
  • Takes an active role in self-development.
  • Performs other duties as needed.

Other information:

  • Certified Pharmacy Technician (CPhT) required
  • 1 year experience in Healthcare/Medical - Pharmacist & Pharmacy Technician  or Retail Pharmacy experience or equivalent 
  • Minimum of 2 years of retail pharmacy experience preferred
