Job Description

The inpatient nurse clinical navigator will facilitate the process of collaboration between the Licensed Providers (LP) and the nursing team and effectively navigate the discharge process for the patient.


  • Participates in daily charge nurse hand-off for the prioritization of patients identified for discharge.
  • Builds relationships with departmental leaders, frontline staff, care management and the licensed providers to enhance the discharge process.
  • Facilitates the “First Touch” Rounds between the LP and the Primary Nurse for the inpatient population in the department.
  • Attends the Communication and Patient Planning Rounds (CAPP) and follow-up with the patients identified for discharge.
  • Collaborates with Care Management and other specialists to coordinate the discharge plan.
  • Provides a point of contact for patients and families on day of discharge to facilitate communication and efficiency.

    Education Requirements:

    • Graduation from a school of professional nursing
    • BSN preferred
    Licensure/Certification Requirements:

    • Licensed to practice as a Registered Nurse in the state of North Carolina.
    • BLCS Certification

    Professional Experience Requirements:
    • One (1) year of nursing experience caring for acute and chronically ill patients or an equivalent combination of education and experience.
