UNC’s Department of Pharmacy is an organization of more than 300 dedicated employees with focused attention on caring for patients of the UNC Health Care system. The department is culturally diverse and has a variety of backgrounds, interests, capabilities, and experience. Additionally, the UNC Department of Pharmacy is nationally recognized for excellence in pharmacy practice and has received numerous ASHP Best Practices Awards in Health-System Pharmacy.

For over 40 years, clinical pharmacy services have played a vital role in the multidisciplinary management of hematology/oncology patients at the North Carolina Medical Center. The Department of Pharmacy Hematology/Oncology Team provides comprehensive care for patients with a variety of hematologic malignancies and disorders and solid tumors in both the ambulatory care and acute care settings with an emphasis on continuity of care.

Summary:
The focus of this position will be on providing comprehensive pharmacist care services, disease management, and specialty medication monitoring and management in the ambulatory care setting to adult patients seen in the CNS/Neuro-oncology clinic. This individual will ensure continuity of care as the patient transitions between acute care and ambulatory care settings, while continuing to pursue best practices in this area. This position will work closely with specialty pharmacy services in the ambulatory care setting as well as the neuro-oncology interdisciplinary team to support adult patients seen in the clinic. Additionally, a responsibility exists for providing experiential instruction to doctor of pharmacy students and UNC Medical Center pharmacy residents.

Reporting Relationships:

This individual will directly report to the Clinical Manager of the Hematology/Oncology Service Line. Additionally, this individual will work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network.

Responsibilities:

The primary job responsibilities include providing comprehensive pharmacist care services and optimizing medication use for patients seen in the CNS/Neuro-oncology clinic through innovative clinical services, ensuring continuity of care by bridging acute and ambulatory pharmacy services, and leading quality improvement and outcomes initiatives.

Clinical responsibilities include (80%)

  • Manage a CPP clinic for 4 clinic days per week which collaborates with physicians and advanced practice providers to see patients and bill for services
  • Serve as a liaison between the Department of Pharmacy, CNS/Neuro-oncology clinic, and Department of Neurology to assure all activities are coordinated and consistent
  • Educate patients and their caregivers about their chemotherapy plan
  • Support medication management goals set forth by the institution or Department of Pharmacy
  • Manage chemotherapy toxicities (oral and IV chemotherapy)
  • Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage treatment plans
  • Coordinate medication-related aspects of transitional care
  • Collaborate with the acute care pharmacist responsible for the Solid Tumor Service and the chemotherapy infusion pharmacy team on policies, procedures, and guidelines associated with this patient population
  • Provide comprehensive medication management for patients receiving oral chemotherapy agents
  • Document patient encounters in the electronic medical record and with documentation of items necessary for facility fee billing
  • Serve as clinical liaison among clinical CNS/Neuro-oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications

Service Responsibilities (10%)

  • The outpatient CNS/Neuro-oncology Clinical Pharmacist Practitioner will serve as a standing committee member on a select group of committees within UNC Medical Center
  • The pharmacist will maintain credentialing as a Clinical Pharmacist Practitioner
  • The pharmacist will contribute to ongoing quality assurance and outcomes initiatives for the Department of Pharmacy, the Hematology/Oncology Division, the Department of Neurology, and UNCMC

Educational Responsibilities (10%)

  • The outpatient CNS/Neuro-oncology Clinical Pharmacist Practitioner will have responsibilities to educate students from the UNC Eshelman School of Pharmacy, PGY1 and PGY2 residents from UNC Hospitals on their experiential rotations in CNS/Neuro-oncology.

Skills and Attributes:

  • Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology
  • Strong oral and written communication, organizational and prioritization skills
  • Ability to work in team environment
  • Ability to generate meaningful contributions related to oncology pharmacy practice and advancement of pharmacy practice.
  • Ability to design and implement projects, collect and interpret data accurately, and prepare project analyses/summaries/manuscripts

Preferred Qualifications:

  • Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
  • Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience
  • Board Certification in Oncology Pharmacy

Licensure/Certification Requirements:
● CPP must by a registered Pharmacist and licensed CPP in the state of NC. Credentialing and Privileging must be completed through the appropriate UNC HCS accrediting body as an APP prior to start date.
Professional Experience Requirements:
● North Carolina registered Pharmacist:. BS Pharmacy or Pharm D. BPS Certification or Geriatric Certification or ASHP residency program + 2 years of experience
● North Carolina Registered Pharmacist:. Pharm D. 3 years experience. 1 certificate program
● North Carolina Registered Pharmacist:. BS Pharmacy. 5 years experience. 2 certificate programs
