Summary:

Responsible for auditing Emergency Department
medical records for nursing and physician documentation, accuracy, and
completeness. Responsible for assigning Evaluation
Management (E/M) level procedure charges for each patient visit, using ACEP
Guidelines, and entry of charges into Meditech. Identifies issues related to any of the above
and forwards findings to the Charge Auditor Coordinator.


Responsibilities:
1. Review 100% ED patient medical records for physician, nurse and registration documentation completeness.
2. Forwards incomplete documents to appropriate physician or clinician.
3. Assigns charge levels and procedure codes using ACEP guidelines.
4. Enters charge level and procedure codes in Meditech.
5. Runs ED log to make sure all charts are assigned a level charge.
6. Notifies appropriate ED staff of any missing charts or components thereof.
7. Educates and orients staff to the charging process and documentation standards for the department.
8. Provides support to Patient Financial Services to assist with billing issues and patient inquiries.
9. Communicates with staff any documentation issues that may arise.
10. Reviews charts for MD completeness prior to forwarding charts to HIM coders. Forwards incomplete charts to MD for completion and tracks information.

Education Requirements:
● Requires graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
Licensure/Certification Requirements:
● Must have current RN compact licensure in the state of North Carolina.
Professional Experience Requirements:
● Requires a minimum of five (5) years experience as a Registered Nurse with at least three (3) years experience in the Emergency Department.
