Summary:
The MDS Coordinator assumes the responsibility and accountability for completion of the MDS and RAPS for each resident of MNC. Identifying areas of potential increased reimbursement is also a vital part of this position. The MDS Coordinator visits residents to ensure maximum care and to ascertain the need for modified services. This position serves adult and geriatric residents.
Responsibilities:
1. Completes MDS and RAPS for each resident and ensures all MDS is transmitted according to schedule.
2. Works closely with rehab and Director of Nursing to ensure maximum reimbursement for each resident.
3. Participates in weekly medicate meeting with rehab services and participates in interdisciplinary care planning.
4. Participates in the identification of changes in health status of residents, which determines the need for reassessment.
5. Reviews charts monthly and audits for summaries, vital signs, weights, and pharmacy profiles; conducts quarterly reviews in compliance with federal regulations.
6. Monitors weekly reports and participates in development of comprehensive care plan for reach resident based on the comprehensive resident assessment and quarterly reviews in compliance with federal regulations.
7. Participates in special projects, research, and nursing education as directed.

● Graduation from an accredited school of nursing.
Licensure/Certification Requirements:
● Licensed as LPN in North Carolina.
Professional Experience Requirements:
● Requires 3-5 years experience in long term care and previous MDS experience.
Knowledge/Skills/and Abilities Requirements:
● Requires management and supervision skills.
