Description

Summary:
The Referral Coordinator assists patients with scheduling follow-on care as referred by a care provider.
Responsibilities:
• Pulls referrals from fax or electronic work cue and logs appropriately for tracking purposes. Forwards referral to physician for review and approval. If referral is not approved, prepares and mails/faxes notification letter to referring provider.
• Once referral has been approved, contacts patient to conduct medical history interview and schedules first appointment. Initiates new patient checklist. Enters patient information into electronic medical record. Verifies insurance information for referral purposes as well as patient’s mailing address. Assists patient with special needs requests such as transportation, interpreter needs, etc. Prepares new patient appointment letter and packet; places it in outgoing mail by end of business day. If unable to reach patient after 2 attempts, prepares and mails notification letter to the referring provider.
• Ensures new patient file is complete and ready for patient’s initial appointment. Coordinates retrieval of patient medical files and other appropriate documentation from referring physician and/or current or past providers as needed. Coordinates completion of a patient consent for release of medical information from providers as appropriate.
• Answers, responds and documents phone calls, requests and questions from patients per policy in a timely manner. Accurately manages and redirects calls as appropriate.
• Assists with office support as requested, including scanning, filing, etc. Provides backup support for patient check-in/check-out as assigned. Offers support to office staff as approved and assigned by management. Performs other duties as assigned.
Qualifications
Required
• Three (3) years of experience working in a healthcare practice or clinic setting.
• Prior experience with patient scheduling and referral coordination.
• Education or demonstrated proficiency with medical terminology.
• Experience with Electronic Medical Recordkeeping (EMR) systems.
• Effective verbal and written communication.
• Experience working in a customer service capacity.
