Summary:
The Licensed Clinical Social Worker (LCSW) provides acute and longitudinal patient care in the outpatient medical office setting. They will work in a team-based environment providing counseling services along with ongoing care management support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient and practice needs. The position will collaboratively enhance the quality of patient management and satisfaction, and promote continuity of care and cost effectiveness by partnering with medical providers, care management, and the entire clinical team. The LCSW must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.


Responsibilities:
• Assesses and documents bio-psychosocial needs and prepares a formal intervention plan to suit.
• Evaluates and identifies possible or actual patient/family crises, providing crisis intervention and emotional support, and creating and facilitating effective plans for resolution.
• Delivers information and short-term therapeutic counseling to patients and their families in relation to social, psychological, and family situations.
• Develops and maintains relationships with outside community health and social agencies. Monitors ongoing and new community resources for meeting patient needs.
• Refers patients and families with appropriate community assistance resources.
• Obtains, interprets and documents necessary information regarding patients and families, including bio-psychosocial history factors, and communicates to interdisciplinary care team.


Other information:
Required

  • Licensed Clinical Social Worker (LCSW) certification
  • Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education.


Preferred

• Experience working in an outpatient / ambulatory clinical care setting
