Summary:
Ensures that all patient encounters are received and prepared for scanning in an accurate and timely manner.


Responsibilities:
1. Verifies receipt of all inpatient and outpatient discharged records from nursing units, ancillary departments, and remote campuses. Follow up on missing records to ensure that they are received in a timely manner according to procedure.
2. Confirms patient name, medical record number, and account number are on every page in the chart. (Front and back.) Processes non-bar-coded documents appropriately. Identifies outside vendor barcodes and corrects applicable pages to ensure proper indexing.
3. Complies with requirements for writing summaries and initiating the Peer Review Process using established criteria, so that no penalties or denials are incurred due to failure to comply.
4. Removes all staples and paper clips and sticky notes. Identifies and separates multi-part forms and scans the original. Separates and sequences perforated forms for scanning purposes. Copies all documents so that they are fed through the scanner without difficulty.
5. Operates and maintains assigned equipment in accordance with specifications so that no damage or undue wear and tear occurs. Accesses, manipulates, enters and prints data using designated computer applications, without appreciable assistance. Immediately reports malfunctioning equipment to appropriate person, if unable to correct.
