Performs comprehensive Care Management Screen/Assessment for all patients per departmental policy and appropriately identifies potential discharge planning and/or psycho-social needs. Partners with physicians and interdisciplinary care team to coordinate discharge plans to ensure all critical elements have been addressed and patient is moved to appropriate next level of care. Utilizes critical thinking skills to proactively identify and appropriately address medically complex and/or psycho-social related discharge barriers. Initiates, facilitates and track referrals for post-acute care to include home health, DME, infusion, rehab placement, hospice and others in a timely manner. Candidate will be knowledgeable of post-acute benefits and regulations for payers. Demonstrates effective communication skills both in documentation and verbal formats. Demonstrates proactive, accurate, and timely assessment. Demonstrates timely response to request for Case Management interventions. An Ideal candidate will have Behavior Health Experience as well. Ideal candidate will be able to work efficiently to cover all assigned areas. Candidate will also demonstrate skill and success with collaborating with Utilization Manager assigned to the Emergency Department
Essential Job Functions:
Psychosocial Assessment and Interventions
Based on preliminary risk screening, assess patient and family psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, adjustment to illness, and ability to cope
Intervene with patients and families regarding emotional, social, financial, and environmental concerns of illness and/or disability; access and mobilize family and community resources to meet identified needs
Provide intervention in cases involving child abuse and neglect; domestic violence; abuse, neglect, or exploitation of the elderly and disabled; and sexual assault
Serve as a resource for and providing appropriate referrals and intervention related to treatment decisions and end-of-life issues
Complex Discharge Planning
Involve patients in all decisions affecting the discharge plan of care
Participate in discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers
Intervene with families exhibiting complex family dynamics that impact directly on patient care and discharge
Screens and coordinates all new skilled nursing facility and Rehab facility referrals.
Complete frequent rounding with the Utilization Manager on cases that can be safely dispositioned from the Emergency Department
Educates patient/family and physician regarding post-acute options and addresses issues of choice.
Competency and skill in communicating with patients, families, hospital staff, and visitors
Demonstrate collaboration, professionalism, and ability to work with all health care providers
Demonstrate ability to work with people of all social, economic and cultural backgrounds and is flexible, open minded and adaptable to change
Demonstrate appropriate problem-solving skills
Demonstrates competency in timely and complete assessments, progress notes, and discharge plans.
Demonstrates proactive, accurate, and timely assessment.
Demonstrates timely response to request for Social Work intervention.
Demonstrates skill in prioritizing case load for
appropriate level of intervention
QualificationsGraduation from a state-accredited school of professional nursing.
