This is professional level work involving the evidence- based evaluation of patient disabilities, planning, and administration of physical therapy treatments. Incumbents complete documentation instruct patients, families, and students and participate in clinical program development and management. Clinician is expected to have involvement in the areas of continuing education, departmental and community activities, outreach education and role of preceptor to students.
Specific Duties include:
Maintains an environment of safety for patients, self and others.
Performing an initial patient evaluation and periodic reassessments for lymphedema, cancer and general patient populations. Using objective tests and measures to determine functional outcomes and to monitor effectiveness of treatment programs, with full consideration of patient safety needs.
Rehab goals are established that are functionally based, measurable, and include time frames. Physician notification of first visit included in chart.
Plans and implements individualized patient care programs through the utilization of selected physical therapy treatments based on patient assessment data.
Documents treatment, response, and progress in patient's chart. Completes all required Medicare and insurance documentation. including certs/re-certs, authorizations, charge and time information, and number of visits. Goals are continuously checked and updated
during documented re-evaluations or reassessments. Discharge summaries completed within 2 weeks of discharge
Educates patient and family about treatment and safety procedures to be continued at home. Patient/family education is documented in chart 100% of time. Documentation contains evidence of patient's understanding, knowledge, or performance of education.
Collaborates with other healthcare team members to assure coordination of patient's plan of care and discharge plan, as appropriate. Patient care information is communicated through conferences, email, or department team meetings as indicated. Therapist re-evals and signs off on patients shared with PTA 100% of time.
Determines appropriate time frame for discharge from physical therapy services based on patient achievement (or lack of progression) of established treatment goals.
Orients, instructs, and directs work activities of assistants, aides, and students.
Plans and conducts in-service training on physical therapy and related topics for staff, students, and community groups. Provides in-services within 6 months of attending continuing education program. Provide at least 2 community education programs per year.
Participates in clinics, conferences, and continuing education programs designed to improve patient care and staff performance. Attend discipline specific in-services at least 90% of the time. Attend at least one continuing education course per year.
Maintains current licensure/certification needed to satisfy hospital standards for employment and state requirements to practice physical therapy to include: physical therapy license, hospital annual safety training requirements, basic life support, and departmental appropriate competencies.
QualificationsGraduation from an accredited Physical Therapy program. Licensed as a Physical Therapist in the state of North Carolina.
