Job Description


Position:  Manager, Care Management


Location:  Main Hospital / Chapel Hill


Hours:  Full Time, 40 hrs/wk; Days


Responsible for day-to-day operations for Care Management Team in the Care Management Department. Provides management direction to Inpatient Social Work Care Managers, Care Managers, Utilization Managers, Clinical Social Workers, Clinical Authorization Specialists, and Operational staff. Accountable for Clinical Outcomes, Human Resources Management, Budget Management and Quality/Process Improvement Initiatives to ensure high level of quality consistent with organizational standards. Reports to the Assistant Director of Clinical Care Management.   


Manages personnel functions for Care Management Department team(s). Interviews and selects candidates for hiring, coordinates job offers with Human Resources, coordinates orientation for new staff, develops work plans, conducts performance reviews, and conducts disciplinary action using hospital and departmental personnel guidelines. Collaborates with Assistant Director and Director in preparation and management of departmental budget. Identifies appropriate staffing levels and assignments for designated Care Management department. Manages daily workflow, tracking issues and making adjustments accordingly. Manages the day-to-day scheduling, triaging and tracking productivity or delegates responsibility to advanced-level staff members. Establishes goals for the department and reviews performance to ensure staff is meeting or exceeding target outcome goals. Works closely with medical staff leadership and other staff to ensure appropriate medical staff involvement in clinical care management efforts. Shares with the Assistant Director and Director the responsibility for collecting, analyzing, evaluating and presenting care management operational data. 


Provides regular communication to staff by leading and/or participating in team meetings and team building activities. Participates in committees promoting effective communication between Care Management and other clinical areas. Role models and leads staff in supporting departmental and institutional expectations in customer service excellence. Leads continuous quality improvement efforts in conjunction with the department Assistant Director and Director. Helps ensure compliance with regulatory requirements related to care management, TJC and Health Care System policies. Conducts audits as needed to monitor quality within care management and develops improvement action plans when indicated.






Bachelor's or Master's degree in Nursing (BSN or MSN) from a state-accredited school of professional nursing or Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW) or related field. Five (5) years of experience in care management, utilization management, discharge planning, care coordination, nursing, or social work required or equivalent combination of education and experience. Two (2) years of demonstrated health care leadership experience is also required.
