Position: Pharmacy Transition Specialist / Medication Histories
Location: Rex Hospital / Raleigh
Hours: Full Time, 40 hrs/wk; Overnight
The medication history technician is an integral member of the Pharmacy and Emergency Department teams. The medication history technician is responsible for working with patients who have been identified for admission to the hospital in order to obtain and document the best possible medication history. Medication history information will be obtained from the patient directly or the patient's family, retail pharmacy or physician office. This role is essential to assure high quality and safe care by reducing medication duplications, omissions and possible errors. The information obtained will be accurately entered into the computer system and it will be used by physicians, pharmacists, nurses, and other healthcare providers to make care decisions during hospitalization, and in discharge planning for post-hospital transitions and care.
Skills required: Accurate work with careful attention to detail. Good clinical judgment to determine accuracy of information obtained. Working knowledge of common outpatient medication doses and dosage forms. Possess the necessary interpersonal and communication skills required to interact with patients, co-workers, and other health professionals. Ability to prioritize work in a high-volume work environment. Ability to adapt to changes in workload and schedule to meet patient needs. Ability to set priorities and solve problems.
Completion of a nine-month Pharmacy Technician Program (or equivalent combination of education, training and experience).
If completion of a Pharmacy Technician Program: No prior experience required.
If a High School Diploma or GED: Two (2) years of experience as a Pharmacy Technician.
Registration with the NC Board of Pharmacy unless enrolled in a PharmD program. CPhT from PTCB or ExCPT from NHA unless enrolled in a PharmD program.
