Respiratory Therapist Hospital Liaison:


Under the supervision of UNC Homecare Specialists Respiratory Supervisor, the Respiratory Therapist Hospital Liaison has the responsibility to collaborate with other members of the healthcare team to carry respiratory and sleep therapy patients through the continuum of care for best possible outcomes while transitioning from hospital to home.  This liaison works in a collaborative effort with hospitalists, hospital respiratory therapists, case management and patients’ nursing care team as a resource to provide appropriate respiratory homecare durable medical equipment and supplies.  This liaison takes an active role in facilitating communication of patient needs/orders for DME to medical team to avoid unnecessary delays in care, functional improvement, timely discharge, and readmissions.  Demonstrates desire to promote the company philosophy, and seeks ways to facilitate helping more patients with transitions of care. 


Key duties include:


Utilize UNCHCS electronic medical record-CPR+ for documenting all RT patient visits and communication,  utilize EPIC for clinical view as needed, communicate patient’s responsibility with DME provided,  communicate integral follow-up to UNC Homecare Specialists dispatch team for home delivery needs, maintain inventory of RT DME at on-site location,  the RT hospital liaison coordinates the delivery and supplies that are needed and educates the patient and/or caregiver on proper use, cleaning, troubleshooting of RT equipment prior to discharge,  The RT hospital liaison ensures regulatory paperwork is completed and submitted,  maintains scheduled appointments,  participates in organizational meetings and in-services within department and on-site at the hospital to communicate features, benefits, and programs and educate staff on the services available.  The RT liaison also  meets all guidelines for UNC Homecare Specialists Respiratory Therapists providing home and office patient visits.  All other duties as assigned.   

Part-time position 

Requirements:  RRT, NC Respiratory License, BLS, NCDL

RT hospital liaison follows UNC Homecare Specialists Respiratory Therapist guidelines including:

Maintains an environment of safety for patients, self and others.
Implements routine respiratory therapy procedures as prescribed by a physician. Deliver, set up, pick up and educate patient in the use of supplies and durable medical equipment.
Performs ECD evaluations per MD order with appropriate patient safety needs in mind. Documents using the appropriate standardized template in CPR+. Communicates effectively to MD with negative results.
Delivers and instructs patients on proper use of oximetry equipment as per MD orders. If applicable, downloads data and faxes MD report with results. Documents using standardized template in CPR+.
Evaluates and reviews order for Trach/Suction equipment for accuracy. Deliver and instructs patient on the safe use of equipment and supplies per MD order. Documents using the standardized template in CPR+. Collaborates appropriate inter departmentally to coordinate set up.
Evaluates and reviews PAP therapy orders for accuracy. Sets pap therapy parameters per MD orders and educates patient on the proper use of equipment. Assures appropriate interface and fitting to meet the needs of individual patients. Assures patient is able to apply interface and use effectively. Documents in EMR using standardized template for pap therapy.
Communicates with patients for timely follow up utilizing to do's for two follow up, and note manager to schedule 30 day follow up with patient. Utilizes the form letter for set up date, 30 day follow-up to communicate with MD via Epic inbasket for notification of needed face to face and results of patient compliance.
Participates in the department’s performance improvement plan.
Utilizes CPR+ to review and/or create delivery tickets using appropriate template, progress notes templates, notes manager reports and TO DO's.
Ensures deliveries are on time and correct. Communicates with patients timely if scheduling conflicts arise.
Provide timely follow up with patients as per departmental compliance guidelines. Manages territory case managing the patients. Calls patients timely to schedule appointments and arrival time.
Limits patient/agency complaints about deliveries from patients or caregivers.
Attends staff meetings and in-services to meet job requirements.
Meets education requirements; reviews and updates competencies per regulatory requirements.
Displays a clean, neat, and professional appearance at all times. Complies with dress code 100% of the time.
Maintains attendance standards by following assigned work hours, limiting absences, and providing proper absence notification. Follow-up clock in clock out procedures, uses time wisely, and limits overtime.
Participates in on call rotation. Responds timely to needs when on call. Coordinates appropriately with dispatch, delivery technicians and /or intake to arrange deliveries. Documents outcomes in patient account.
Performs routine troubleshooting of equipment to assess needs of patient.

Provides education and instruction to patient/caregivers on related home care respiratory equipment being provided.













Experience One to two years previous experience and/or training desired.

Education Associate's or Bachelor's degree from an accredited school of Respiratory Care preferred.

Licensure RRT or CRT with general adult care experience required.

This position is employed by NC Health (Rex Healthcare, Inc., d/b/a NC Health), a private, fully-owned subsidiary of UNC Heath Care System. This is not a State employed position.
