This is a full time, FLSA exempt, 40 hours /week M-F position


The position of Nursing Care Coordinator works with the unit based physician team and functions as the Physician Team Nurse Care Coordinator (PTNCC).  This position is filled by a registered nurse (RN) with experience in care of pediatric inpatients. PTNCC report directly to the nurse manager of the primary unit the service places patients. The PTNCC work under the direction of the Section Chief of Hospital Pediatrics to improve the overall efficiency and productivity of the inpatient services. PTNCCs are assigned to inpatient teams and directly supervised in daily work by the attending physician assigned to that team. The main responsibility of the PTNCC is to coordinate and facilitate all aspects of patient care from admission to discharge and to serve as source of continuity for the patient and family as other provider’s turnover. Efforts are focused on patient satisfaction and safety as well as enhanced communication between patients, families, physicians, nurses and other staff.

 

Specific Duties to the Nurse Care Coordinator for unit based teams:

  • Participate in team rounds with focus on continuity of care and discharge planning.
  • Participate in and represent team at Care Progression Rounds on pediatric units bringing knowledge of medical milestones for discharge, barriers to discharge, estimated discharge dates.
  • Communicate with floor nurses changes in status or plans for patients.
  • Perform continuity of care activities including obtaining outside medical records, imaging, and laboratory results.
  • Coordinate inpatient studies and services (sedations, imaging, audiology, etc.)
  • Work with case management for safe and efficient discharge by:

  1. Identifying barriers to discharge early in hospitalization
  2. Scheduling follow-up appointments for clinic visits, studies/procedures and therapies
  3. Assist providers in completion of home health forms
  4. Communicate with primary care and other outpatient patients
  5. Assist with patient education- including review of After Visit Summary (AVS) for accuracy and appropriateness for patient/family.
  6. Liaison with outpatient nurses and services to enhance transitions from outpatient to inpatient to outpatient.
  7. Serve as point of contact for issues that arise for patients shortly after discharge- especially to ensure appropriate outpatient follow-up plans are in place.
  8. Actively participate (and lead) quality and process improvement activities to enhance the safety and quality of care provided.
  9. Assist in coverage for other inpatient team if/when other WTC absent.  

 

Qualifications

Graduation from a state-accredited school of professional nursing. Licensed to practice as a Registered Nurse in the state of North Carolina. Five (5) years of professional nursing experience preferably in the care of children and adolescents.

This is a State position employed by UNC Health Care System.
