A Clinical Pharmacist Practitioner (CPP) functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients in collaboration with a supervising physician and healthcare team.
The focus of this position will be on providing comprehensive pharmaceutical care and disease management in the ambulatory care setting to adult patients seen by the Outpatient Oncology Palliative Care service. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents in support of the Partnership in Patient Care.This individual will directly report to the Clinical Manager of Hematology/Oncology Clinical Pharmacy Services. This individual will work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacist within the UNC Ambulatory Pharmacy Care Network. They will work collaboratively with the multidisciplinary Outpatient Oncology Palliative Care Team
Duties and Responsibilities
The primary job responsibilities include providing comprehensive pharmaceutical care and optimizing medication use for patients seen by the Outpatient Oncology Palliative Care service. The pharmacist will provide innovative clinical services, ensuring continuity of care by bridging acute and ambulatory pharmacy services, and leading quality improvement initiatives.
Clinical Responsibilities include (90%):
·Serve as a drug information resource for palliative care team members
·As an integral member of the Outpatient Oncology Palliative Care Service, supportive care management could be provided to patients in all adult hematology/oncology settings for a goal of 8 half-days per week.This service will ensure that patients have appropriate supportive care management throughout the continuum of their care. The CPP will work closely with other Oncology Palliative Care Service personnel to deliver high quality symptom management through serving as an APP with prescriptive authority for a variety of cancer symptoms including, but not limited to:
Educational and Administrative responsibilities (10%)
·Educating students from UNC Eshelman School of Pharmacy, PGY1 and PGY2 residents from UNC Medical Center on experiential rotations in outpatient hematology/oncology
·Provide didactic and clinical education for palliative care medical fellows
·Serving as a standing committee member on a select group of committees within UNC Medical Center
·Responsible for all medication management goals set forth by the institution or Department of Pharmacy
·Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
·Completion of an Oncology (PGY2) Pharmacy Residency, Palliative Care (PGY2) Pharmacy Residency, or equivalent experience
·Board Certification in Oncology Pharmacy
·Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology and palliative care
·Strong oral and written communication, organizational and prioritization skills
·Ability to work in team environment
·Development of manuscripts for submission from collaboration with colleagues
·Ability to design and implement projects, collect and interpret data accurately and prepare project knowledge base in Hematology/Oncology
● Bachelor's degree in Pharmacy.
● CPP must by a registered Pharmacist and licensed CPP in the state of NC. Credentialing and Privileging must be completed through the appropriate UNC HCS accrediting body as an APP prior to start date.
Professional Experience Requirements:
● North Carolina registered Pharmacist:. BS Pharmacy or Pharm D. BPS Certification or Geriatric Certification or ASHP residency program + 2 years of experience
● North Carolina Registered Pharmacist:. Pharm D. 3 years experience. 1 certificate program
● North Carolina Registered Pharmacist:. BS Pharmacy. 5 years experience. 2 certificate programs
