UNC Health’s Advanced Care at Home is seeking a Virtual Hospitalist to work in a team environment (Physicians, APP’s, RNs and Paramedics) to provide hospital level, virtual care to patients in their own homes.
Qualifications • MD/DO • Prior experience in hospital medicine required • Board Certified/Board Eligible in internal medicine or family medicine • Committed to providing high quality, evidenced based care to a diverse patient population • Prior experience with Epic EMR recommended Responsibilities • Screen and admit patients into the program • Provide daily virtual visits from the clinical command center • Coordinate plan of care with a multidisciplinary team • Supervise NPs • Collaborate with hospitalists, specialists and ancillary services at UNC Medical Center and UNC Rex • Participate in a call rotation to cover night call from home • Advance care planning
UNC’s Advanced Care at Home will replicate the capabilities of the brick and mortar hospital and deliver this care to patients in the comfort and convenience of their own homes. Under the program, UNC Health physicians and nurses will monitor and communicate with patients in their homes 24/7 from a clinical command center located in Chapel Hill, NC. The virtual team will deploy a network of rapid response home based services to patients as needed. Patients will receive in person assessments by nursing, paramedics and NPs, as well as virtual assessments by nurses and physicians. The program is seeking to hire physicians to start in the summer of 2021. “Acute Care at Home reflects UNC Health’s commitment to innovative care as we work to improve the health and well-being of all North Carolinians,” said UNC Health CEO Wesley Burks, MD. “Technology in enabling us to provide the right care to patients in a way that is safe and helps them recover where they are most comfortable. As Acute Care at Home availability expands across North Carolina through ongoing conversations with federal, state and commercial payers, we expect more and more patients will choose to receive high-acuity care at home.”
