Job Summary: The Patient Navigator functions as a member of the UNC Sickle Cell Team to ensure continuity of care for patients and families affected by sickle cell disease. The Patient Navigator will bridge the gap between clinical staff and patient by working in tandem with the medical team and by providing individual support and assistance in navigating the health care system. In this role, the patient navigator will follow the patient throughout the health care continuum. Based upon the individual needs of the patient and caregivers, the patient navigator will provide local and resources for transportation, parking, medical team communication, and emotional support.


  1. Coordinates and facilitates care for patients with sickle cell disease and other hemoglobinopathies and provides emotional support.
  2. Facilitates New Patient scheduling by insuring all relevant documents have been received.
  3. Manage missed/cancelled visit list, in conjunction with providers at weekly meeting.Assess and record barriers to follow up for missed clinic visits. Assist with rescheduling.
  4. Call new patients within 24 hours of visit to check in regarding follow up appointments and needs.
  5. Assist in connecting patients to services for support as well as help initiating certain programs such as transportation, Medicaid, medical housing, contact with NC Sickle Cell Educators, and other community resources (ie: social work, financial counseling)
  6. Help identify and maintain materials and resources for new patients as well as for existing patients as conditions change.
  7. Builds relationships with multidisciplinary teams including but not limited to Gynecology, Physical Therapy, Psychiatry, Pulmonology, Cardiology, Nephrology, Hepatology, Laboratory and Radiology. Help coordinate appts for patients on same day for those patients who live distance from clinic. Fax orders to local outpatient labs and or radiology per patient preference.
  8. Develop and maintain a calendar for our DPS patients to ensure that patients have telehealth visits every 3-6 months and at least one face to face visit per year.
  9. Coordinate with clinic staff, provider, and DPS to ensure that DPS patients have appt scheduled 6 months prior to release, 3 months prior to release and a phone/video/in person visits 2 weeks post release.
  10. Develop and maintain a weekly hydroxyurea safety lab calendar and place reminder calls to patients so that labs are drawn in a timely fashion. Maintain active telephone notes in chart as related to communication with patients. Fax needed orders to patient preferred lab site.
  11. Review exchange list from apheresis and review appt needs. Notify provider for orders needed. Make sure orders are active for patients.
  12. Participate in clinics to facilitate building rapport with patients and families as well as to ensure real-time care coordination efforts in patients who need individualized scheduling assistance.
  13. Attend pediatric review meeting weekly
  14. Help the pediatric program review patient list to maintain compliant follow up clinic appointments (estimated 2 days every 6 months)
  15. Help the peds program review TCD compliance (estimated 1-2 days every 6 months).
  16. Help track the newborns to add to patient list and for billing purposes.
  17. Helps with scheduling new transition patients from peds to adult clinics

● No licensure or certification required.
Professional Experience Requirements:
● Five (5) years of clerical experience of increasing variety and/or complexity.
● If Bachelor's degree, one (1) year of clerical experience of increasing variety and/or complexity
● If Associate's degree, three (3) years of clerical experience of increasing variety and/or complexity.
Knowledge/Skills/and Abilities Requirements:
