Job Summary:

Project Coordinator is part of the UNC Health Alliance (“UNCHA”) Program Management Office (“PMO”) and is responsible for day-to-day activities of assigned projects for UNCHA.

The Project Coordinator – Pediatric Population Health will support the North Carolina Integrated Care for Kids (NC InCK) model as a lead for InCK’s team of Integration Consultants. NC InCK's goal is to improve the health and well-being of North Carolina youth and families insured by Medicaid or Health Choice by creating a new way to care for children and youth that integrates services for children, including physical and behavioral health, food, housing, early care and education, juvenile justice and legal aid.

The Project Coordinator – Pediatric Population Health’s working title on the NC InCK team is Lead Integration Consultant. This position will report directly to the NC InCK Managing Director and will support the work of 14 Integration Consultants (IC), licensed clinical professionals with experience in coordination of care activities based in organizations across the service model. The Lead Integration Consultant is responsible for operationalizing the IC workflow, standards, and processes as InCK launches its new model in January 2022.

This role is responsible for collaboration with key functions across UNCHA and other UNC Health entities. Job function responsibilities include: project planning and administration, communicating effectively at all levels of the organization, financial management, monitoring and reporting progress, and building strong relationships in a matrix environment.

Description of Job Responsibilities:

Lead Integration Consultant Responsibilities:

  • Oversee daily operations of the IC team across partner sites in conjunction with multidisciplinary teams, including supporting design and improvement of InCK’s service model.
  • Track and document the activities of IC at sites across the child health service organization, identifying areas of need, including resource and personnel allocations for care management activities.
  • Convene and lead meetings for 10-15 IC across all core child service areas to standardize processes and establish relationships to build value and enhance care provision for the model.
  • Manage and liaison with the Lead Integration Consultant at Duke Health, the Managing Director at NC InCK and other external teams to design, develop and implement IC programs through aligned efforts and directing personnel.
  • Coordinate and collaborate with external partners such as Community Care of North Carolina (CCNC), Duke PHMO, and NC Medicaid Standard Plans on behalf of InCK.
  • Integration Consultant Responsibilities (for reference):

  • Facilitate a single point of contact for integrated care coordination or case management across healthcare and social services sectors for children requiring multiple services or who are at risk of out of home placement.
  • Support the generation of policies and processes for a Shared Action Plan across healthcare and social services sectors for InCK attributed children tiered to higher risk for improved whole-child service coordination.
  • Identify new opportunities to build capacity for improved child health and well-being across child core service healthcare and social service sectors through regular convening’s of Integration Consultant team.
  • Train existing front-line coordinators in cross-sector child and adolescent specific resources and best practices.

    Education Requirements:
    ● Associate's degree in an appropriate discipline or equivalent combination of education, skills, and experience.  Bachelor’s degree preferred in a clinical or business-related field such as Nursing, Social Work, Therapy, Business, or Community Health related fields.

    Licensure/Certification Requirements:
    ● No licensure or certification required.
    Professional Experience Requirements:
    ● If an Associate's degree, three (3) years of experience in an administrative support role and/or projects-related role. If a high school diploma/GED, seven (7) years of experience in an administrative support role and/or projects-related role.  

    Knowledge/Skills/and Abilities Requirements:

    ·Ability to plan and administer programs and direct program activities and staff is strongly preferred.

    ·Customer service experience through patient or public contact preferred.

    ·Active user of electronic medical records software strongly preferred.

    ·Supervisory experience in a care management environment preferred.
