Position Summary:
The focus of this position will be on providing comprehensive pharmaceutical care, disease management and transitions of care in the acute care setting to adult patients seen on the inpatient gynecology oncology service. This individual will ensure continuity of care as the patient transitions from the inpatient to the outpatient setting and vice versa, while continuing to pursue best practices in these areas. This position will be responsible for transitioning care to the Ambulatory Pharmacy Care Network in the acute care setting. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents in support of the Partnership in Patient Care.
Job Duties:
The primary job responsibilities include providing comprehensive pharmaceutical care and optimizing medication use for patients on the inpatient medical oncology service as well as providing transitions of care coordination in collaboration with ambulatory clinical pharmacist practitioners and the UNC Ambulatory Pharmacy Care Network.In addition, the selected candidate will serve to develop innovative clinical services, and leading quality improvement initiatives.
Medical Oncology Service responsibilities (80%)
Participate in daily patient care rounds, providing support to medical and nursing staff, including order verification, profile review, chemotherapy order review, and clinical consultation as needed, and ensuring appropriate and optimized medication therapy management for patients on the service
Evaluate, monitor, and redesign medication regimens to ensure positive patient outcomes
Complete Therapeutic Drug Monitoring:
Antibiotics, anticoagulants, anti-fungals, immunosuppression, methotrexate and other medications per department of pharmacy standards
Review all chemotherapy orders for accuracy and appropriateness prior to order verification by Cancer Hospital Infusion Pharmacy (CHIP)
Collaborate with medical team to optimize and modify treatment plan orders as outlined by policy
Ensure adherence to hospital policies, including but not limited to policies regarding chemotherapy, biotherapy and immunotherapy
Review Patient Admission Medication Histories
Coordinate Patient Medication Discharge Process:
Review discharge medication reconciliations
Coordination of prescriptions with outpatient pharmacies
If filled through UNC Central Outpatient Pharmacy, coordinate with technician to ensure all required medications have been delivered to the patient
Provide discharge counseling and complete appropriate documentation
Ensure adequate medication payer coverage by facilitating prior authorizations and payer appeals
Coordinate with UNC’s Shared Services Center Specialty Pharmacy and the Medication Assistance Program (MAP) to verify patient’s insurance requirements and ensure timely processing of all prior authorizations.
Collaborate with Ambulatory Care Network to provide direction for available financial assistance resources upon transitions of care.
Provide pharmacy support for rapid responses and codes
Provide comprehensive services through oncology-directed weekend and holiday staffing
Assist in development and implementation of innovative disease management and medication use programs
Preferences:
Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience
Board Certification in Oncology Pharmacy

Education Requirements:
● Bachelor's degree in Pharmacy.
Licensure/Certification Requirements:
● Eligible for licensure as a Registered Pharmacist in NC. Licensure from NC Board of Pharmacy required within 6 months of orientation date or sooner if required for operations.
