Provides care of patients by optimizing medication use and drug distribution services.
TEAM SUMMARY AND REPORTING RELATIONSHIPS
The Medication Management and Optimization team serves UNC Health and the Department of Pharmacy, including hospitals, clinics, retail pharmacies, and other care settings, across all of North Carolina. The Medication Management and Optimization team provides drug information, formulary management, policy, clinical guideline, and other expertise to internal and external partners with the mission to support optimal pharmaceutical care for all patients at UNC Health. The team includes 4 Clinical Pharmacist Specialists and 2 Pharmacy Student Interns. The reports to the Clinical Manager, Medication Management and Optimization. The Clinical Specialist, Medication Management and Optimization is eligible for partial remote work and partial on-site work, in accordance with UNC Health policies.
KEY RESPONSIBILITIES
The key responsibilities of the Clinical Specialist, Medication Management and Optimization will include the following:
·Lead and co-chair a UNC Health System Pharmacy and Therapeutics Subcommittee(s) by preparing materials, facilitating meetings, and implementing decisions
·Maintain and design an evidence-based hospital and clinic medication formulary that promotes therapeutic appropriateness, medication safety, and financial stewardship
·Monitor and review medication formulary compliance, including adjudication of non-formulary drug requests
·Identify, analyze, design, and execute system-level medication-use optimization initiatives using a coordinated approach that incorporates
·Work collaboratively with multidisciplinary stakeholder groups across the health care system and with external partners to ensure success of initiatives (eg, prescribers, pharmacists, nurses, Pharmacy Analytics and Outcomes team, Pharmacy Supply Chain Center team, Pharmacy Revenue integrity team, group purchasing organization, pharmaceutical vendors)
·Oversee routine functions of the drug information call center, including identifying, critically analyzing, and summarizing medication research
·Develop and maintain multidisciplinary and Department of Pharmacy medication-use policies and clinical guidelines
·Participate in and contribute to quality assurance and other programs that maintain regulatory compliance (eg, TJC, NC Board of Pharmacy)
·Serve as a primary preceptor of pharmacy students and PGY1 and PGY2 pharmacy residents
·Deliver didactic and other learning experiences within relevant UNC Eshelman School of Pharmacy courses, as needed
·Perform other functions, as needed
SKILLS AND ATTRIBUTES
The Clinical Specialist, Medication Management and Optimization should be able to efficiently work in a fast-paced environment and demonstrate abilities to complete the following:

POSITION PREFERNCESS
·Relevant ASHP-accredited PGY1 and PGY2 residencies (eg, Health-System Pharmacy Administration, Drug Information, Medication-Use Safety and Policy), or equivalent experience

Education Requirements:
● Bachelor's degree in Pharmacy.
Licensure/Certification Requirements:
● Eligible for licensure as a Registered Pharmacist in NC. Licensure from NC Board of Pharmacy required within 6 months of orientation date or sooner if required for operations.
Professional Experience Requirements:
● No prior experience required.
