The Patient Care Coordinator (PCC) is a Registered Nurse who is responsible for coordinating patient care and activities that ensure patient care services and supplies are rendered during their work shift. He/she represents the Hospital and Nursing Administration in their absence and acts as a resource for patients, families, staff, physicians and community agency personnel regarding Hospital and Nursing policy, procedures, philosophy and mission. He/she also serves as a resource for Nursing Clinical Practice.
1. Coordinates patient care and guides nursing staff in problem solving and decision making during a worked shift and has no more than four documented validated complaints per appraisal period.
2. Assists the Admission Department to place new patients on appropriate nursing unit according to diagnosis, acuity and the nursing units’ activity.
3. Is knowledgeable in interpreting the nursing department acuity/staffing system and uses this information as a guide to assist and coach unit personnel with staffing adjustments.
4. Maintains frequent communication with nursing units in order to assess patient care activity and needs on individual units and as a department.
5. Assists the unit charge nurses to locate staffing coverage when a sudden increase in census/acuity occurs or scheduled staff is unable to work.
6. Coordinates efforts, in collaboration with the Engineering Department, to transport essential staff to work during inclement weather.
7. Coordinates activities to provide sleeping arrangements and food for staff during inclement weather or other unusual events that require staff to work or be on site beyond the normal work hours.
8. Is accessible to readily assist staff nurses and charge nurses to make assessments regarding individual patient status changes and evaluation of need for immediate physician intervention.
9. Demonstrates general knowledge of job duties to be performed by employees on shift usually worked.
10. Represents Hospital and Nursing Administration during their absence with no more than two documented complaints by Administration/Director of inappropriate/inconsistent representation during the appraisal period.Demonstrates current up-to-date knowledge of hospital and nursing department mission, policies and goals and acts as a resource for nursing and others.Acts in Administrative role in emergencies including Internal and External Disasters, National Disaster Drills, etc. until the administrator is available on site.Acts as a patient advocate and is a positive role model to effect change and care that is patient focused.Intervenes with patient/family complaints in absence of Department Manger/Administrator.Coaches and teaches staff when observing unsafe practices or deviations in care based on established standards or procedures.Acts as a liaison between nursing staff and Administration, role modeling positive acceptance of change and actively listening to staff concerns and problems. Encourages solution seeking activities and shares concerns with management as deemed necessary.Acts as a liaison between nursing staff and medical staff in order to promote safe, appropriate patient care.Clarifies physician orders and utilizes the appropriate chain of command to advise of perceived patient care problems, inappropriate actions toward patients or inappropriate orders given by medical staff.Receives and distributes patient valuables following hospital policy/procedure.
11. Stores and provides telephone equipment for translation services that hospital staff can utilize for non-English-speaking patients/families.Demonstrates a sound theoretical nursing knowledge base and serves as a clinical resource to nursing staff.Attends procedure and equipment inservices, serves as a resource in operating basic patient care equipment and is familiar with specialty department equipment and its location.Maintains basic clinical skills in order to directly assist nurses/physicians for limited periods of time or in emergency situations when extra help is temporarily needed with no more than three documented complaints regarding unwillingness to assist.Attends mock codes/drills and assesses staff performance as requested.Utilizes knowledge of available staff resources or own expertise to assist hospital staff to perform difficult procedures.Attends yearly hospital updates and completes Patient Care Coordinator competency updates.Processes all admissions and discharges bodies from the morgue, completing documentation according to established procedure.Assures in-hospital deceased patients are transported to the morgue as necessary.Assists the Medical Examiner as requested to examine and process individuals being held in the hospital morgue.Notifies the N.C. Organ Procurement Organization and obtains relevant information for this organization pertaining to DOA entering the hospital morgue.
12. Uses effective communication and collaboration skills with no more than four documented, validated complaints of poor communication per appraisal period.Appropriately notifies Administrator/Director “on call” of information/problems that impact on a positive hospital image, hospital/public relations or potential legal risks.Communicates patient/family or perceived care problems to the Department Manager/Risk Manager/Administration in writing and verbally in an objective and timely manner after interviewing involved individuals.Collaborates and negotiates effectively in order to accomplish appropriate and safe staffing adjustments without creating unnecessary stress and anxiety.Employs a supportive, therapeutic manner in communicating with patient/families/caregivers.Defuses potentially disruptive situations with physician/staff, encouraging private reasonable discussions to resolve conflicts.Gives concise, complete report of shift activities to the oncoming PCC. Documents significant variances in patient care, sentinel events, department/patient/family problems, staffing adjustments, department activity per unit and morgue status on the PCC Report Form.Keeps the Director informed of unusual events or problems.Assists in providing bereavement counseling and resources to families of patients and community members experiencing sudden death or severe disability.Assists families of deceased by providing information regarding available services or processes concerning funerals, transportation, death certificates, etc.Communicates appropriately with the media, emergency medical/law enforcement personnel, community leaders, families and patients, assisting readily as requested or as policy allows.Organizes and coordinates activities that ensure services, equipment and supplies are available and rendered for those departments unavailable on a 24 hour basis.Screens calls for help with malfunctioning equipment and case manager/social worker services and notifies “on-call staff” as appropriate for engineering, case management, social services and biomedical personnel.Assures supplies and/or equipment are procured for requesting nursing units as needed.Notifies “on-call” materials management personnel when supplies and/or equipment are unavailable in the facility.Assures patient charts are obtained from Medical Records as requested by physicians during hours Medical Records is closed.Assures patient food trays and nutrients are obtained as needed when dietary is closed.Checks patient prescribed medication orders with pharmacy clerk against pulled drugs during non-pharmacist hours. Obtains patient prescribed medications from the stock drug cart or pharmacy when pharmacy is closed? documents appropriately.Notifies the surgical “on-call team” for emergency surgery when requested by the attending surgeon and schedules nonemergency surgery with the OR “Booker on-call” during hours the operating room is closed.Notifies appropriate personnel and assists with activities in transporting patients to another facility by helicopter.
13. Obtains copies of records, sends records by fax to health care facilities as requested after obtaining permission of the patient and assures copies of records are transported with patients transferring to other facilities.Coordinates activities for the Organ Procurement Team to obtain a surgical suite for harvesting donated organs.Provides hospital tour for residents providing physician weekend coverage.Obtains meals and assures rooms are provided and in readiness for physicians, weekend resident physicians and surgery or other hospital staff as needed to meet patient care needs.
● Bachelor’s degree in Nursing.
● Licensed to practice as a Registered Nurse in the state of North Carolina.
Professional Experience Requirements:
● Three (3) years of progressive experience in related field.
