Summary:
The focus of this position will be on providing comprehensive pharmacist care services, disease management and specialty pharmacy support. This individual will ensure continuity of care as the patient transitions between acute care and ambulatory care settings, while continuing to pursue best practices in this area. This position will support specialty pharmacy services in the ambulatory care setting to adult patients seen at these clinics. This position will ensure that clinical coverage can be provided during vacancies within the team as well as supporting administrative time for the Lead Pharmacist as well as other team members as needed. Additionally, a responsibility exists for providing experiential instruction to doctor of pharmacy students and UNC Medical Center pharmacy residents.
Responsibilities:
Clinical responsibilities include (80%)
- Medication Therapy Management
- Manage an outpatient hematology/oncology clinic; collaborates with physicians and advanced practice providers to see patients and bill for services
- Serve as a liaison between the Department of Pharmacy and the Division of Hematology/Oncology to assure all activities are coordinated and consistent
- Educate patients and their caregivers about their chemotherapy plan
- Support medication management goals set forth by the institution or Department of Pharmacy
- Manage chemotherapy toxicities
- Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage treatment plans within Epic electronic health record (EHR)
- Coordinate medication related aspects of transitional care
- Collaborate with the acute care pharmacists and cancer hospital infusion pharmacy team caring for the hematology/oncology patients on pharmacy related policies, procedures, and guidelines
- Provide comprehensive medication management for patients receiving oral chemotherapy agents • Document patient encounters in the EHR including items necessary for facility fee billing
- Specialty Pharmacy Services:
- Serve as a clinical liaison among the clinical oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications
Service Responsibilities (10%)
- The outpatient Clinical Pharmacist Float will serve as a standing committee member on a select group of committees within UNC Medical Center
- The pharmacist will contribute to ongoing quality assurance initiatives for the Department of Pharmacy, the Hematology/Oncology Division, and UNCMC
Educational Responsibilities (10%)
- The outpatient Clinical Pharmacist Float will have responsibilities to educate students from the UNC Eshelman School of Pharmacy and PGY1 and PGY2 residents from UNC Medical Center on their experiential rotations in adult hematology/oncology rotations
SKILLS AND ATTRIBUTES
- Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology
- Strong oral and written communication, organizational, and prioritization skills
- Ability to work in team environment
- Ability to generate meaningful contributions related to hematology/oncology and advancement of pharmacy practice.

Education Requirements:
● Bachelor's degree in Pharmacy.
Licensure/Certification Requirements:
● Eligible for licensure as a Registered Pharmacist in NC. Licensure from NC Board of Pharmacy required within 6 months of orientation date or sooner if required for operations.

● No prior experience required.
