Summary:

As a part of UNC Cancer Care, it provides state of the art cancer care in a convenient community setting to the residents of Lenoir and surrounding communities. Services offered at the McCreary Cancer Center include medical oncology, radiation oncology chemotherapy and blood product infusion, and laboratory services. There is a full service infusion pharmacy onsite that prepares and dispenses infusion therapies for patients being treated in the infusion center. UNC’s Department of Pharmacy is an organization of more than 300 dedicated employees with focused attention on caring for patients of the UNC Health Care system. The department is culturally diverse and has a variety of backgrounds, interests, capabilities, and experience. Additionally, the UNC Department of Pharmacy is nationally recognized for excellence in pharmacy practice and has received numerous ASHP Best Practices Awards in Health-System Pharmacy. For over 40 years, clinical pharmacy services have played a vital role in the multidisciplinary management of hematology/oncology patients at the North Carolina Medical Center. The Department of Pharmacy Hematology/Oncology Team provides comprehensive care for patients with a variety of hematologic malignancies and disorders and solid tumors in both the ambulatory care and acute care settings. This position will provide similar high level clinical services at the McCreary Cancer Center site.

 POSITION SUMMARY:The focus of this position will be on providing comprehensive pharmaceutical care, disease management and specialty pharmacy clinical support in the ambulatory care setting to adult patients seen at the oncology clinic at the McCreary Cancer Center. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents as able. The role offers the potential for some virtual/remote work. 

REPORTING RELATIONSHIPS:This individual will directly report to the Clinical Manager of the Hematology/Oncology Service Line. Additionally, this individual will work closely with the Hematology/Oncology clinical and operational pharmacists at the UNC Medical Center and McCreary Cancer Center as well as the Specialty pharmacists within the UNC Carolina Care Pharmacy Network. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:The primary job responsibilities include providing comprehensive pharmaceutical care and optimizing medication use for patients in the general oncology clinic as well as providing specialty pharmacy clinical support for oncology clinic patients. This position will provide innovative clinical services, educate and manage patients on IV and oral chemotherapy, ensure continuity of care by bridging acute and ambulatory pharmacy services, and lead quality improvement initiatives. 

Effort Allocation will be as follows: 

Clinical responsibilities include (80%)• Medication Therapy Management • Manage a CPP clinic for 8 templated clinic half-days per week which collaborates with physicians, advanced practice providers and nurses/nurse navigators to see patients and bill for services • Serve as a liaison between the Department of Pharmacy and the McCreary Oncology Clinic to assure all activities are coordinated and consistent • Educate patients and their caregivers about their chemotherapy plan • Manage chemotherapy toxicities • Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage treatment plans • Coordinate medication-related aspects of transitional care • Collaborate with other oncology pharmacist on policies, procedures, and guidelines associated with this patient population • Provide comprehensive medication management for patients receiving oral chemotherapy agents • Document patient encounters in the electronic medical record and with documentation of items necessary for facility fee billing • Specialty Pharmacy Services • Refer patients seen in the clinic to UNC’s Carolina Care Pharmacy Network for outpatient medications, including oral oncolytics. • Identify and refer eligible patients for enrollment in specialty pharmacy services • Refer patients to the Medication Assistance Program (MAP) for benefits investigation to facilitate timely processing of prior authorizations or financial assistance resources 

Service Responsibilities (10%)• Represent the pharmacy clinical oncology group on relevant committees aimed at process or quality improvement, patient safety, cancer care or any other oncology or pharmacy departmental initiatives. • Maintain credentialing as a Clinical Pharmacist Practitioner • Contribute to ongoing quality assurance initiatives for the Department of Pharmacy, the Hematology/Oncology Division, UNCMC and/or McCreary Cancer Center

 Educational and Administrative Responsibilities (10%)• Educate students from UNC Eshelman School of Pharmacy, PGY1 and PGY2 residents from UNC Medical Center on experiential rotations in outpatient oncology. • Provide didactic and clinical education to students, residents, and other team members • Responsible for all medication management goals set forth by the institution or Department of Pharmacy 

OTHER SKILLS AND ATTRIBUTES:• Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology • Strong oral and written communication, organizational and prioritization skills • Ability to work in team environment • Development of manuscripts for submission on collaboration with colleagues • Ability to design and implement projects, collect and interpret data accurately and prepare project knowledge base in Hematology/Oncology • Ability to critically evaluate workflows, processes and/or clinical services in order to generate and maintain a practice for continuous quality improvement • Ability to establish and maintain quality metrics to capture and demonstrate impact 

QUALIFICATIONS & REQUIRED EXPERIENCE 

Required:• Licensure or license-eligible as a pharmacist in the State of North Carolina • Doctor of Pharmacy Degree from an accredited college or university or equivalent degree • Eligibility to obtain Clinical Pharmacist Practitioner designation in the State of North Carolina 

Preferred: • Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience • Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience • Board Certification in Oncology Pharmacy Compensation will be competitive and commensurate with experience.
